Dietitian – PACE Program

Salary: $63,000–$93,000/year + $5,000 Sign-On Bonus (paid after 90 days)

Schedule: Full-time | Onsite or Hybrid (onsite 5 days/week preferred)

We’re seeking a dedicated Dietitian to join our PACE (Program of All-Inclusive Care for the Elderly) team in beautiful Farmville, VA. In this rewarding role, you’ll support older adults in maintaining their health, independence, and quality of life—while being part of a collaborative, values-centered healthcare organization.

What You’ll Do

As the primary Dietitian on site, you will play a vital role in shaping the nutritional care of our participants. Responsibilities include:

• Delivering medical nutrition therapy as part of an interdisciplinary team
• Conducting comprehensive assessments and creating individualized nutrition care plans
• Providing education and counseling to participants and caregivers
• Documenting all care in the electronic health record
• Supporting participants primarily on-site, with occasional home visits within a one-hour radius
• Working independently with strong clinical judgment and adaptability
